About the Position Job Title: Case Manager Profession: Registered
Nurse Specialty: Case Management Duration: 13 weeks Shift: Day
Hours per Shift: 8 hours Experience: Minimum of 2 years of acute
care hospital case manager experience performing care coordination
and discharge planning is required. Previous charge experience is
preferred. Travel experience is required. License: A valid state
nursing license is required. A pending license is accepted but must
be licensed upon start date. Certifications: Basic Life Support
(BLS) certification is expected. Must-Have: Experience with care
coordination and discharge planning in an acute care hospital
setting is essential. Knowledge of assessing patients and
developing treatment plans is necessary. Experience with
computerized documentation and proficiency in Microsoft
applications is required. Description: Perform care coordination
and discharge planning for patients. Facilitate family meetings and
develop treatment plans. Arrange durable medical equipment, skilled
nursing facilities, and ambulance transport independently.
Participate in interdisciplinary support and work collaboratively
with social services. Knowledge of drug/alcohol abuse treatment
options, advanced directives, and protective services processes is
necessary. Maintain effective communication with patients and
families regarding treatment plans and community resources. Must
commit to working weekends and covering departmental needs as
required. Ability to float between various facilities within the
state is expected. Strong organizational skills and the ability to
handle crisis situations effectively are important in this role.
