Job Description:
A company is looking for a Case Manager, Registered Nurse
(Remote).
Key Responsibilities
Assess, plan, implement, and coordinate case management activities
for members to evaluate their medical needs
Develop proactive care plans to enhance members' short and
long-term health outcomes
Conduct comprehensive assessments using various data sources to
address co-morbid conditions and facilitate referrals to clinical
resources
Required Qualifications
Active and unrestricted Registered Nurse (RN) licensure in South
Carolina or compact licensure in the state of residence
3+ years of clinical experience
2+ years of case management, discharge planning, or home healthcare
coordination experience
Associate's Degree in Nursing (required); Bachelor's Degree
(preferred)
Certification as a Case Manager (preferred)
